Hampshire Hospitals NHS Foundation Trust has a self-referral service for hearing assessments.

Who is Eligible?
Patients aged 18 years or over who have:
 Hearing difficulty
 Tinnitus which is bothersome
 No ear pain or discharge
 No episodes of dizziness or vertigo
 No rapid onset hearing loss

Children aged 8 months or above where parents are concerned about a possible hearing problem.

If you meet the above eligibility criteria, please contact the surgery (via telephone, eConsult or face to face) and we will provide you with a self-referral form.  This form should be completed and sent back to the audiology department by email or post.  The service will then contact you to arrange an appropriate appointment.
