COVID Travel Pass

If you require proof of vaccination or recovery from COVID this needs to be obtained via the NHS App or from 119.  Stockbridge Surgery is unable to provide letters for patients regarding vaccination status or proof of recovery.

Do you need vaccinations?

Our Nurses can provide comprehensive travel health advice and arrange for appropriate immunisations. Please complete a travel health form for each traveller and ensure it is returned to the surgery at least 8 weeks before your date of travel.  We prefer to receive forms electronically.   Vaccines need time to take effect and some may require a course over several weeks.

We offer a full travel advice and immunisation service. If you are travelling abroad please complete and submit the travel form in plenty of time (at least 8 weeks) before your journey.

Travel Vaccination Prices

Travel advice is covered by the NHS; however some vaccinations and prescriptions are only available privately. Payment for non-NHS services must be made before any appointment is booked.
